Todd,
Here are some quick and dirty ways to create your W-2 overlay.
1. you could go to the IRS website and download their W-2 form in .pdf format, and then use the free IBM Windows AFP Driver to create the overlay. (This method is guaranteed to be within their specs - as it is theirs!)
2. you could use a scanner and scan the current form and create a .pdf or .rtf file and use the free IBM Windows AFP Driver to create the overlay.
3. you could hire a consultant (like me :o) to do it for you.

We currently use AFP on an IBM mainframe to print our company employee W2
forms. We use preprinted forms and use AFP to place the data inside the
preprinted boxes. Due to issues with inconsistent forms and the cost of
preprinted forms, I am investigating other options.
I have searched and not found any information regarding the requirements I
would need to follow if I were to create an overlay to print the boxes and
other text that currently exists on our preprinted forms. Is anyone doing
this, using an overlay to print the boxes and other text? Does anyone know
where I can find information about the requirements (box size, font, etc.)
to print these forms? I only need to print the B, C and 2 copies (4-up
format) to give to our employees. Our submission to SSA is done
electronically, so I don't need to do anything with Copy A.
I could use a preprinted W2 form and duplicate it in an overlay, but I
thought there would be some sort of spec sheet existing for instructions
for W2 employee copies.
