RE: Insomnia  Matthew Hancock

I used to have dreadful sleep because of RLS. After trying various meds
including pergalide, sinimet and quinine sulphate (now banned I am told),
Requip has been my wonder drug for about 18 months now. Amazingly I
achieved this by taking just 1mg in the evening. Recently I have begun to
show some tolerance with more disturbed nights. If this doesn't settle, the
Prof is happy for me to increase the dose.

I'm a bit late coming back on this, Robin, but I've been out of touch.

I have been taking Requip since February, when I was taken off Pergolide
because of its possible effects on the heart. I was prescribed 1 mg at
night only, increasing the dose if necessary. This didn't help, even by
the time I was taking 4 mg. I got a good night's sleep - (4 mg had a
sandbagging effect!)- but I too was getting attacks during the day,
mostly in the afternoon. I was sure, from my experiencce with Pergolide,
that I should be able to take daytime doses too, but it wasn't till I
saw a consultant neurologist in June that I started doing this.

I now take .5 mg in the morning if necessary, which isn't often; .5 mg
with my lunch as I am almost certain to have an afternoon attack; and
2.5 mg at night. The night-time dose is enough to make me feel sick
too, and I am still uncertain whether I prefer to take it with my meal
(6.30-7.00), and risk feeling sleepy straight after, or to leave it
until I go to bed, when I risk having an attack between eating and going
to bed. It surprises me that your evening dose does not appear to kick
in for about 3 hours. I wonder if increasing it a small amount might
make the difference.

I think your doctor is probably right about the licensing of Requip. My
consultant says it has only been licensed for one night-time dose up to
4 mg, because proper tests have not been done for dosing throughout the
day (although that is how it is prescribed for Parkinsons). But he also
said one could be flexible, and suggested I try reducing my night-time
dose from 3mg, which I was on when I saw him, and that I could increase
my lunch-time dose towards 3 mg if needed (which it is not at the
moment).

Requip has been the best solution for me although I have only just
enjoyed the benefits having increased my dose to two tablets in the
evening. This has given me much better nights as I only have to get up
two or three times and then only for a short while - generally.

The one problem I have found is that I have to take the tablets with
food otherwise I feel sick. Taking them with my evening meal (6.30pm)
means that I only walk around my bungalow until about 10.00pm when the
tablets appear to kick in.

I would like to know what dose of Requip other people are on. I suffer
RSL sometimes in the mornings, through the afternoon and then again in
the evening. I have been down to about half an hour of sleep through
the night. The only relief is to keep on the move!

Robin

PS - I am new to this group and welcome the chance to exchange ideas
regarding RSL
