That looks good and should be very useful. I will try
to get some other forms to work with such as the
Express Scripts that we can convert to .pdf . I was
looking again at pdffill a while back. I think it has
an option to use a scanner and then you can create
your own boxes and buttons. IIRC, pdfbox could then
generate the .fdf or .xfdf file. Is that right? That
might be a low cost way to make some more useful .pdf
forms.

I've just about completed the work on getting the new
CMS 1500 form to print out. I pretty much had to make
a complete duplication of the old system and them
modify it in parallel with the old system. That way I
still have the old system, which is still working for
MDOn-line and OfficeAlly for electronic, but have a
new system I can play with for the new version of the
CMS1500 form without messing up the old one.
